Order A Test

Before you can order tests, you must first register with your contact and company information. If you have already registered, sign in and order your first test immediately by following these steps:

Note: Unless you have pre-approval from Mayo Clinic Laboratories, your company is financially responsible for all tests that you order.

Order Your First Test

After you register and sign in, follow these steps to order your first test:

  1. Find the test that you want to order in the Test Catalog.
  2. Click the Order This Test button, which is located next to the test name.
  3. Provide the following information:
    • Primary business contact
      This contact provides information about testing needs, volume, and services, and answers general questions about the account.
    • Billing and finance contact
      This contact receives electronic invoices and is responsible for making payments.
  4. Click Submit.
  5. Print and complete the test requisition forms PDF to ship with the specimen.

Your first order will be processed and an account number will be assigned to you. Repeat these steps for each test that you order until you receive your account number.

When your account is ready, you will receive a Welcome email with your account number and detailed information about advanced ordering options, test results notification, billing, and other services.
